Buying a New LIC Policy
New PurchaseProposal Form
Standard LIC proposal form filled completely
NRI Questionnaire
Special form for NRIs. available from LIC agent or branch
Passport Copy
Self-attested copy of valid Indian passport (all pages with stamps)
OCI Card Copy
If OCI cardholder. self-attested copy (both sides)
Visa / Work Permit Copy
Proof of legal residence abroad. self-attested
PAN Card
Copy of PAN card (mandatory for policies above ₹50,000 premium)
Aadhaar Card
If available. not mandatory for NRIs but helpful
Address Proof (Abroad)
Utility bill, bank statement, or rental agreement showing overseas address
Address Proof (India)
Indian address proof. Aadhaar, passport, or utility bill at Indian address
Income Proof
Salary certificate, employment letter, or tax returns from country of residence
Photographs
2-3 recent passport-size photographs
Age Proof
Passport serves as age proof; alternatively birth certificate
Bank Details
NRE or NRO account details. cancelled cheque or bank statement
Medical Reports
If required based on age/sum assured. from LIC-empanelled doctor
Renewing / Paying Premiums
RenewalPolicy Number
Your LIC policy number for identification
NRE/NRO Bank Account
Premiums must be paid from NRE or NRO account
Online Payment
Can pay via LIC portal (licindia.in) using net banking/cards
ECS Mandate
Set up auto-debit from NRE/NRO account for hassle-free payments
Reviving a Lapsed Policy
RevivalRevival Application Form
Available from LIC branch
All Unpaid Premiums + Interest
Payment of all dues. LIC will calculate the exact amount
Passport Copy (Attested)
Current valid passport
Declaration of Good Health
Self-declaration or medical exam based on lapse duration
Power of Attorney
If someone in India is handling revival on your behalf
Filing a Claim
ClaimClaim Form
Form 3783 (death claim) or discharge voucher (maturity)
Original Policy Bond
Original policy document. or indemnity bond if lost
Death Certificate
For death claims. attested by Indian Embassy if death occurred abroad
Claimant's ID Proof
Passport, Aadhaar, or PAN of the nominee/claimant
Bank Account Details
Cancelled cheque of claimant's bank account for NEFT credit
Medical Certificate
Cause of death certificate from attending doctor/hospital
FIR / Legal Documents
For accidental death. FIR, post-mortem report
